I have been using AIOSEO for several years on mine and client sites. I recently found a need to exclude pages, but am not having any luck with this feature.
My exclude list:
/locations/,/locations/memphis-acupuncture/,/locations/nashville-acupuncture/,/locations/jackson-acupuncture/
The issue is that I have "huntsville" in the page title format and I don't want this propagated onto the pages for alternate locations.
AIOSEO seems to work fine otherwise. Any ideas?

I'm glad you're finding this feature useful. We just added it in the new release this past week.
I'm not sure what you're asking. What is AIOSEOP doing that it shouldn't be, what is it not doing that it should be?

Exclude isn't excluding.

Someone else just reported this as well this weekend. There was a bug in 1.6.x where excluded pages were being excluded, but the checkbox wasn't displaying it as excluded, so of course if you saved it again it would save as non-excluded.
This has been fixed in version 1.6.6 which I released today.

I still have issues with this. I deleted the excludes, saved and reentered, but the titles are still appearing on excluded pages.
Is there supposed to be check box to enable/disable this feature?

To make this easier, I disabled all plug-ins except:
Akismet
All in One SEO Pack
Gallery Widget
Google Analytics for WordPress
Google XML Sitemaps
Random Image Widget
No change, titles are still appearing on excluded pages.

Disable all plugins except All in One SEO Pack, switch to the default theme, and then test.
